Facility is Free of Accident Hazards

In this scenario, the facility failed to ensure that each resident received adequate supervision and assistance to prevent choking and/or aspiration. Could this happen at your facility?

    To prevent aspiration I would recommend the following:
    1. All pureed diets are pureed by dietary staff and checked by dietary supervisor before delivered to resident.
    2. All thickened liquids should be prepared by staff that have been tested and demonstrated that they can prepare the drinks at the proper consistency. Or purchase pre-thickened drinks in the different consistencies.
    3. Be specific on caregiver instructions on how to feed, Ex: If resident is coughing or gagging on thin liquids or pureed food try alternating with a thicker consistency if ordered.
    4. If resident is being given food and drink at the consistency ordered and the resident is still demonstrating symptoms such as coughing or gagging then the MD should be notified on what steps to take such as order Swallow evaluation for another consistency.
