F-323 Free of Accident Hazards/Supervision/Devices:
OXYGEN SAFETY

In this Federal Monitoring Survey, the facility failed to ensure resident safety. Based on observation, interview and record review, the facility failed to ensure the resident environment remained free of accident hazards for one (R35) of one resident who utilized oxygen while sitting under an operating hair dryer in the facility beauty shop in the sample of 12. This deficient practice resulted in an Immediate Jeopardy (IJ) that began on 2/9/10. The Administrator was notified of the IJ situation on 2/9/10. The IJ was removed and corrected on 2/9/10. The Administrator developed a policy, in-serviced all staff on the policy and implemented the policy.

What actions would you and your staff members take to prevent this from occurring in your facility?

Keep yourself and the oxygen supply at least five feet (1.5 meters) away from: any fire, stove, oven, grill, or other heat source or open flame & electrical appliances that make sparks & elements that produced high heat, such as an electronic igniter, toaster, space heater, hair dryer or electric motor
    - Do not use flammable aerosol sprays (e.g. spray paint, hair spray) near the oxygen supply because they may spontaneously burn
    2. Develop a policy and procedure to guide staff what to do in situations in which residents could not be without oxygen and want to have the beautician work on their hair.
    3. Resident only to enter the beauty shop with oxygen in place after all electrical equipment has been turned off.
    4. Resident will receive services by alternate means: scissors only (no clippers) and allow to air dry.
    5. In-service staff on policy and procedure including the beautician.
    6. Post sign on beauty shop door, "STOP NO OXYGEN"
